An accident at the Fermi National Accelerator Laboratory (Fermilab) was investigated in which two electricians received serious flash burns as a result of an electrical fault and subsequent electrical arc blast while working on a 480 VAC motor control center. In conducting its investigation, the Accident Investigation Board (the Board) used various analytical techniques including accident analysis, barrier analysis, and event and causal factor analysis.
