Lessons Learned Report Electrical Safety During Excavation and Penetrations

This report provides lessons learned from a series of independent oversight inspections of electrical safety practices during excavations and penetrations.  The lessons learned were collected during environment, safety and health inspections conducted at four Department of Energy (DOE) sites in 2004.  The inspections were performed by the DOE Office of Independent Oversight and Performance Assurance’s Office of Environment, Safety and Health Evaluations, within the Office of Security and Safety Performance Assurance.
