An accident at the Idaho National Laboratory (INL) was investigated in which a technician sustained a serious injury to his right hand while operating a table saw. In conducting its investigation, the Accident Investigation Board (the Board) used various analytical techniques, including accident analysis, barrier analysis, change analysis, and event and causal factor analysis.
