Headquarters Facilities Master Security Plan - Chapter 11, Incidents of Security Concern
Whenever a failure to properly protect classified matter is detected, it must be formally reported and investigated. This chapter describes the procedures for doing so. It also covers the process for administering security infractions to those responsible for a security incident. This chapter is maintained by the HQ Security Incidents and Infractions Program Manager in EHSS-41.
