This report documents the results of an independent review of construction quality at the Department of Energy's (DOE) Salt Waste Processing Facility at the Savannah River Site.  The on site part of the review was conducted May 14-18, 2012, by the DOE Office of Safety and Emergency Management Evaluations, which is within the DOE Office of Health, Safety and Security.  The focus of the review was piping and pipe supports.  The review team also observed the installation of two tanks in dark cells and, subsequent to the onsite visit, reviewed procurement records for nuclear grade piping and piping system components.
