This report documents the results of an independent review conducted May 15-17, 2012, at the Department of Energy's (DOE) Pacific Northwest National Laboratory by the DOE Office of Safety and Emergency Management Evaluations, which is within the DOE Office of Health, Safety and Security. The review consisted of a shadow assessment of the DOE Office of Science's review of the laboratory's Radiochemical Processing Laboratory Criticality Alarm System.
