This report provides the results of an inspection of environment, safety, and health management at the Department of Energy's (DOE) Lawrence Livermore National Laboratory. The inspection was conducted in October and November 2004 by the DOE Office of Independent Oversight and Performance Assurance's Office of Environment, Safety and Health Evaluations, within the Office of Security and Safety Performance Assurance. A second volume of this report provides more detailed results of the inspection in four technical appendices.
