On October 28, 2013, the U.S. Department of Energy (DOE) Office of Independent Enterprise Assessments’ Office of Enforcement issued an Enforcement Letter (WEL-2013-03) to LVI Services, Inc. related to a hazardous energy control event that occurred when an excavator struck an energized 480-volt overhead power line at DOE’s Portsmouth Gaseous Diffusion Plant.
