This report documents the results of an independent oversight review of safety significant Blast Door Interlock and personnel Door Interlock Systems at the Pantex Plant. The Pantex Plant is operated by Babcock & Wilcox Technical Services Pantex, LLC under contract to the National Nuclear Security Administration Production Office. Independent Oversight also reviewed the performance of Department of Energy oversight to provide input for its evaluation of the effectiveness of the Federal assurance capability. The Independent Oversight review was performed during July 1-2 and July 22-31, 2013, by the U.S. Department of Energy’s (DOE) Office of Safety and Emergency Management Evaluations, which is within the DOE Office of Health, Safety and Security.
