Lessons Learned from Targeted Reviews of Implementation Verification Review Processes at Department of Energy Nuclear Facilities
This report summarizes and analyzes the results of independent reviews of IVR and associated processes at eight DOE sites conducted by Independent Oversight from August 2011 to September 2012. The purpose of the Independent Oversight review was to evaluate the processes and methods used for verifying the implementation of safety basis hazard controls and periodically re-verifying that the controls remain in place.
