On June 2, 2005, the U.S. Department of Energy (DOE) issued a nuclear safety Enforcement Letter to Lawrence Livermore National Laboratory related to quality assurance deficiencies associated with the handling of a cracked high explosive that occurred at the Pantex Plant.
