On June 21, 2004, the National Nuclear Security Administration issued a Preliminary Notice of Violation (EA-2004-05) to the University of California for violations of 10 C.F.R. Parts 830 and 835 associated with an August 5, 2003, event resulting in radiological uptakes by multiple personnel; a September 27, 2003, event that exposed five worker to toxic vapors; and programmatic issues in the radiological protection program at the Los Alamos National Laboratory.
